Admissions Checklist

  • Complete the Online Application by selecting Apply Online and then Admissions Online.

  • Submit completed documents along with the $150.00 nonrefundable application fee.

  • Application must include 1 copy of your child’s current (within the last 3 years) full psychological evaluation.

  • Application must include a copy of IEP (Individualized Education Plan) if applicable.

  • Application must also include a photo of your child.

  • Records Release form must be signed and submitted to your child's school in order to transfer school records.                                                                                                                                                                                                                                   

  • Provide the school with a current Georgia immunization form #3231. This is required before registration will be final.

  • Provide the school with a copy of the original birth certificate.

  • Provide current Hearing and Vision test results.

  • Provide Speech/Language and Occupational Therapy test results if applicable.

  • GRACEPOINT does not accept new applicants in grades 7 and 8.
